The Dane County CIT Program provides community based clinical Case Management services specializing in supporting those with severe and persistent mental illness. With a client-centered wraparound services approach CIT is able to effectively meet clients where ever they are at in the community. With psychiatry, nursing, peer support, and a comprehensive team of case managers the CIT team is able to truly meet the needs of those they serve. All referrals are processed through the CCS Portal which can be accessed here.
